C | 报告显示，经过5年接续帮扶，脱贫县农村居民人均可支配收入从2020年的12588元增加到2025年的18627元、年均增速达8.2%。乡村基础设施和公共服务水平大幅提升，脱贫地区和其他地区之间的发展差距不断缩小。
D | 报告明确，常态化帮扶各负其责、各司其职的责任体系，上下联动、统一协调的政策体系，精准识别、精准帮扶的工作体系基本建立。
E | 监测帮扶机制有效运行，及时将有返贫致贫风险的农户识别为防止返贫致贫对象，均落实针对性帮扶措施，做到早发现、早干预、早帮扶。产业、就业帮扶持续加力，完善到户产业奖补及帮扶产业利益联结机制，全力以赴稳住脱贫劳动力就业岗位，脱贫人口务工规模超过3200万人；重点区域稳步发展，新确定160个国家乡村振兴重点帮扶县和390个省级乡村振兴重点帮扶县，继续统筹资源予以倾斜支持，一批补短板促发展项目有效实施。
F | 报告说，持续巩固拓展脱贫攻坚成果还面临如返贫致贫风险仍将长期存在、欠发达地区发展基础仍然薄弱等困难挑战。 报告明确，下一步要把常态化帮扶纳入乡村振兴战略统筹实施，将防止返贫致贫对象和欠发达地区作为过渡期后常态化帮扶对象，以有力有效的开发式帮扶增强内生发展动力，以健全完备的社会保障体系兜牢民生底线，做到政策供给长效化、措施方法常规化、管理方式高效化、要素保障制度化，持续巩固拓展脱贫攻坚成果，扎实推进乡村全面振兴。
